REGISTRATION & PROXY ACCESS

REGISTER ONLINE USING THE NHS WEBSITE

We are pleased to announce you can now register with us online. This is a paperless service. Registration will be faster and easier if you have your personal information ready.

You do not need proof of address or immigration status, ID or an NHS number to register online with a GP.

Please also complete our New patient - practice agreement form that can be found in the section below. You should be on our clinical system within 7 working days.
 

REGISTER AS A NEW PATIENT

Our register is open and we are accepting registration requests from patients living in our catchment area. Please click on Practice Catchment Area to see if you can register with us.  Registrations are usually completed within 7 working days of receipt.

If you are living in our area and wish to register with us, please complete all of the following forms by clicking on each of the links below in our Registration Pack. The proxy form is for a guardian/parent to have access for a child under 11 years old to book their appointments and order medication.

  • GMS1 Registration Form
  • Adult Registration Form (the Child Registration Form if under 16 years old)
  • Online Access Registration Form
  • New Patient-Practice Agreement Form

Please print off and complete all the appropriate forms, depending on the age of each patient wanting to register. You can either then pop them in the post or email them to us at swlicb.centralsurgery@nhs.net with a copy of the following documentation;

One photographic proof of identification, either a photo driving licence or passport

AND

One proof of your current address, which can either be a recent utility bill (no less than 2 months old), or a mortgage / bank statement or a letter from your landlord or a rental agreement.

PLEASE NOTE THAT WHERE THERE ARE SEVERAL PEOPLE FROM ONE HOUSEHOLD REGISTERING YOU MUST EMAIL EACH PERSON'S REGISTRATION APPLICATION WITH SUPPORTING EVIDENCE SEPARATELY. 

All our new patients are offered a health check with a member of our clinical team to check that all required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

YOU MUST ALLOW AT LEAST 7 WORKING DAYS FOR YOUR ACCOUNT TO BE REGISTERED. PLEASE ENSURE THAT YOU HAVE COMPLETED ALL THE RELEVANT FORMS INCLUDED IN THE REGISTRATION PACK BEFORE SUBMISSION. INCOMPLETE APPLICATIONS WILL DELAY YOUR REGISTRATION.

TEMPORARY PATIENT REGISTRATIONS

If you are ill while away from your usual gp surgery and staying with someone in our catchment area we may be able to provide you immediate and necessary treatment, depending on our appointment availability on the day and if it is deemed that your medical problem is urgent. You may be signposted to 111, the urgent care unit at Teddington, Kingston Chambers Extended Hours Access Hub or 999.

If we can see you, you will need to register as a temporary patient and may have access up to 14 days.  

 

PROXY ACCESS

You can request proxy access for another patient, subject to passing identity checks and receiving their written permission, which could enable you to book appointments, order repeat medication, view their immunisation history and their test/investigation results.

We advise that you read the information leaflet which you can download by clicking on the blue bar below, before starting to complete an application form. Once you are sure you have the credentials to apply, you will need to download and complete the Proxy application form which is obtained by clicking on the 2nd blue bar and email/present the completed form to the practice.

You should hear from us within 14 working days, regarding your application. If you require any assistance with this process, please do contact us.

MOVING OUT OF THE PRACTICE CATCHMENT AREA

Central Surgery has a strict registration policy which states that if a patient moves out of the practice's catchment area the patient will not be able to stay with us and will have to register with a GP in the area where they move to.

This is so the surgery remains non discriminatory and is fair to all those patients who live in the practice’s catchment area to be able to get an appointment.
